  • Case managers help soon to be mothers, current mothers, or children (birth to age 20) who have Medicaid and:

    • have health problems

    • are at high risk of health problems

  • A case manager will assist you with:

    • Assessment: Evaluate needs, strengths, and risks.

    • Care Planning: Develop individualized care plans.

    • Referrals and Resource Coordination: Connect with healthcare providers and community services.

    • Advocacy: Protect rights and navigate the system.

    • Education and Support: Provide guidance on prenatal care, parenting, and healthy lifestyles.

    • Monitoring and Follow-up: Regularly track progress and reassess.

    • Crisis Intervention: Assist in managing emergencies or crises.

    • Documentation and Record-keeping: Maintain accurate and confidential records.
